How to register

To register 15th AMDS and AMSN Student Chapter, please click "Register" button from the navigation to register an account. Once your account is created, you can start fill the registration form by by clicking "Login" button.

Once you logged in, you can start to fill the online form. Here's the guide to fill in the form.

1. In the Name field, please type your full name and your title (if any). This will be used in the certificate later.
   
2. Choose the correct Gender from the radio button.
   
3. Please fill the Date of Birth by choosing the corresponding drop down box.
   
4. Select the Country, and fill in the Whatsapp Number with the format: country code + phone number.
   
5. Please type your institution/univeristy in the University field.
6. Choose the Type of participant, and choose the additional cultural tour option (if any). If you are an AIPKI member, please also choose the hotel from the drop down field.
7. Please specify your food allergy in Food Allergy field, if you don't have any allergy, please type "None".
8. Specify your food preference in Food Preference field, for example: "Halal food, no meat, vegetarian, glutten free, etc".
9. In the Flight Arrival field, please input your Flight ID with the format: Airline + Flight ID
10. Please also specify the Flight Arrival Date too.
11. Also do the same in the Flight Departure and Flight Departure Date field.
12. Write the simple Biosketch in the field. Please also include your name.
13. Choose the Knowledge in Bahasa Indonesia from the drop down box.
14. Please upload yourPhotograph in the field.

The registration will be closed on 14 May 2025

After you complete the form, click the Submit button. You will be redirected to the User Panel.

In the User Panel, you can check your data, and your total fees must be paid (if any). Please scroll until the end of the page, and upload the documents needed.
